What is Form 9 and when does it need to be completed?

Form 9 is a mandatory periodic audit tool for service accessibility compliance under Israel's Equal Rights for Persons with Disabilities Regulations (Service Accessibility Accommodations), 2013. The audit must be conducted every five years by a licensed accessibility consultant and covers four areas: physical accessibility, information and digital access, service procedures, and staff training. Any deficiencies found must be corrected within six months.

What Does Form 9 Include?

The form is divided into four main sections:

  1. Physical Accessibility Accommodations – Evaluating the accessibility of the physical environment, including entrances, pathways, restrooms, signage, and accessible parking.
  2. Information Accessibility – Making information available to the public, including forms, publications, websites, and telephone systems.
  3. Adapting Policies, Procedures, and Practices – Reviewing service procedures, including the option to receive service with a companion, a service animal, and without waiting in line.
  4. Staff Training and Accessibility Coordinator – Verifying that employees are aware of their obligations and have been trained accordingly, and that an accessibility coordinator has been appointed within the organization.

How Is the Review Conducted?

The review is typically carried out by a licensed accessibility consultant for services, who completes the form according to official guidelines. The form includes columns indicating the topic being assessed, the nature of the check, the relevant regulation, the outcome (compliant / non-compliant / not applicable), and any exemptions or special conditions.

Once the form is completed, any deficiencies identified must be remediated within six months. The form and accompanying certificates are kept on file by the organization and must be made available to the public upon request.

Important to know: If the service provider is part of a chain or network, the review must be approved by a licensed accessibility consultant.
